Who:Girls who will be entering the 3rd – 9th grade in the Fall of 2017

Take Route 2 West from Route 128 or East from Route 495
• From Route 2 westbound, go north (right turn) on Fiarhaven Road. Entrance to facility is 100 yards on right.
• From Route 2 eastbound, go north (left turn) on Sudbury Road. Turn Right on Fairhaven Road. Entrance to facility is 3 blocks ahead on the left.
What To Bring:Soccer cleats, shin-guards, soccer ball with name on it, water bottle, lunch, sun screen, rain gear, footwear for lunchtime (no cleats allowed inside the building), change of socks recommended.
Registration Requirements: Completed on-line registration form, including an electronic copy of a physical examination within the past 24 months and a copy of immunization records, and payment in full will be required to participate. Pursuant to state law, no camper will be allowed to participate until all forms have been received. Registration is first come, first served. Camp has sold out in past years.
Medical Care: A full-time athletic trainer will be on duty at all times to administer care for athletic injuries and ailments. For more serious problems, campers will be transported to Emerson Hospital.
Refund Policies: Cancellations made 3 or more weeks prior to the beginning of the session will receive a full refund less a $50 processing fee. Alternatively, the full registration amount can be applied to the following year’s registration. Refunds will not be granted with less than 3 weeks notice unless there is a medical reason. Refund requests for medical reasons will be considered on a case-by-case basis.
Massachusetts Department of Health: This camp must comply with regulations of the Massachusetts Department of Public Health and be licensed by the Concord Board of Health.
